We are here to support families seeking diagnostic evaluations for pre-school aged children
who have not yet entered kindergarten.

How to Begin

To start the process, please click Request an Evaluation Intake Form above and gather the following documents to be uploaded.

Note: Only a parent or legal guardian may complete this form.

Required Documents:

Student's Birth Certificate
Parent/Guardian official Picture ID (Driver's license, state ID, passport)
Legal Guardianship documents (if applicable)
Vision and Hearing Screening results (if available)
Proof of Seminole County Residency:
- A copy of a current utility bill or initial order for service
- And one of the following:
  - Current Homestead Exemption
  - Property taxes
  - Deed
  - Title
  - Current lease or notarized letter from landlord

Next Steps

Once you have completed the evaluation request intake form and uploaded the required documents, you will be contacted to schedule an appointment.

Important Notes

A separate evaluation request intake form must be submitted for each child.
Please be as thorough and accurate as possible in your responses.
Please note that incomplete or missing documentation may delay timely processing of your request.
The person bringing the child to the appointment must be a legal guardian.
